Insurance Code § 1551.059","heading":"CERTIFICATE OF COVERAGE.","body":"The board of trustees shall provide for issuance to each employee or annuitant participating in the group benefits program a certificate of coverage that states:\n(1) the benefits to which the participant is entitled;\n(2) to whom the benefits are payable;\n(3) to whom a claim must be submitted; and\n(4) the provisions of the plan document, in summary form, that principally affect the participant.\nAdded by Acts 2001, 77th Leg., ch. 1419, Sec. 3, eff.
