Insurance Code § 1701.002","heading":"APPLICABILITY OF CHAPTER TO FORMS OF CERTAIN DOCUMENTS.","body":"This chapter applies to the form of the following document:\n(1) a policy, contract, or certificate of:\n(A) accident or health insurance, including group accident or health insurance;\n(B) medical or surgical insurance, including group medical or surgical insurance;\n(C) life or term insurance, including group life or term insurance;\n(D) endowment insurance;\n(E) industrial life insurance; or\n(F) fraternal benefit insurance;\n(2) an annuity or pure endowment contract, including a group annuity contract;\n(3) an application attached or required to be attached to the policy, contract, or certificate; or\n(4) a rider or endorsement to be attached to, printed on, or used in connection with the policy, contract, or certificate.\nAdded by Acts 2003, 78th Leg., ch. 1274, Sec. 5, eff.
