Insurance Code § 4101.058","heading":"LICENSE FORM.","body":"(a) The commissioner shall prescribe the form of a license issued under this chapter.\n(b) A license must contain:\n(1) the adjuster's name;\n(2) the address of the adjuster's place of business;\n(3) the date of issuance and the date of expiration of the license; and\n(4) the name of the firm or insurer with whom the adjuster is employed at the time the license is issued.\nAdded by Acts 2003, 78th Leg., ch. 1274, Sec. 7, eff.
