Insurance Code § 544.451","heading":"DEFINITION.","body":"In this subchapter, \"health benefit plan\" means a plan that provides benefits for medical, surgical, or other treatment expenses incurred as a result of a health condition, a mental health condition, an accident, sickness, or substance abuse, including an individual, group, blanket, or franchise insurance policy or insurance agreement, a group hospital service contract, or an individual or group evidence of coverage or similar coverage document. The term includes:\n(1) a small employer health benefit plan or a health benefit plan written to provide coverage with a cooperative under Chapter 1501;\n(2) a standard health benefit plan offered under Subchapter A or Subchapter B, Chapter 1507; and\n(3) a health benefit plan offered under Chapter 1551, 1575, 1579, or 1601.\nAdded by Acts 2007, 80th Leg., R.S., Ch. 730 (H.B. 2636), Sec. 3B.023(a), eff.
