Insurance Code § 843.076","heading":"APPLICATION.","body":"(a) Any person may apply to the commissioner for and obtain a certificate of authority to organize and operate a health maintenance organization.\n(b) An application for a certificate of authority must:\n(1) be on a form prescribed by rules adopted by the commissioner; and\n(2) be verified by the applicant or an officer or other authorized representative of the applicant.\nAdded by Acts 2001, 77th Leg., ch. 1419, Sec. 1, eff.
