Labor Code § 21.4032","heading":"AUTHORIZED DISCLOSURE.","body":"An individual or the legal representative of an individual may authorize disclosure of genetic information relating to the individual by a written authorization that includes:\n(1) a description of the information to be disclosed;\n(2) the name of the person to whom the disclosure is made; and\n(3) the purpose for the disclosure.\nAdded by Acts 1997, 75th Leg., ch. 1215, Sec. 1, eff.
