Occupations Code § 1702.112","heading":"FORM OF COMPANY LICENSE.","body":"The department shall prescribe the form of a company license. The company license must include:\n(1) the name of the company license holder;\n(2) the name under which the company license holder is to operate;\n(3) the company license number and the date the company license was issued; and\n(4) a photograph of the company license holder, affixed to the company license at the time the company license is issued by the department.\nActs 1999, 76th Leg., ch. 388, Sec. 1, eff.
