Occupations Code § 351.263","heading":"LOST OR DESTROYED LICENSE.","body":"The board shall issue a license under this chapter to a license holder whose license has been lost or destroyed on:\n(1) submission of an affidavit stating:\n(A) that the license has been lost or destroyed;\n(B) that the person making the affidavit is the person to whom the license was issued; and\n(C) any other information required by the board; and\n(2) payment of a fee for issuance of a duplicate license.\nActs 1999, 76th Leg., ch. 388, Sec. 1, eff.
