Occupations Code § 353.152","heading":"REQUIREMENTS FOR CONTACT LENS PRESCRIPTION.","body":"(a) A contact lens prescription must contain, at a minimum:\n(1) the patient's name;\n(2) the date the prescription was issued;\n(3) the manufacturer of the contact lens to be dispensed, if needed;\n(4) the expiration date of the prescription;\n(5) the signature of the physician, optometrist, or therapeutic optometrist or a verification of the prescription described by Section 353.1015;\n(6) if the prescription is issued by an optometrist, specification information required by Texas Optometry Board rule; and\n(7) if the prescription is issued by a physician, specification information required by Texas Medical Board rule.\n(b) The Texas Optometry Board and the Texas Medical Board may adopt rules regarding the contents of a prescription for contact lenses.\nActs 1999, 76th Leg., ch. 388, Sec. 1, eff.
