Special District Local Laws Code § 8801.156","heading":"APPLICATION FOR PERMIT.","body":"(a) A person must submit an application to the board to obtain a permit under this chapter.\n(b) The application must state:\n(1) the name and address of the person requesting the permit;\n(2) the location and wellhead elevation of the well or proposed well;\n(3) the amount of water being withdrawn or proposed to be withdrawn; and\n(4) any other information necessary for the board to control and prevent subsidence in the district.\n(c) The board shall set a reasonable fee for processing an application.
