Transportation Code § 623.080","heading":"CONTENTS OF PERMIT.","body":"(a) Except as provided by Subsection (b), a permit under this subchapter must include:\n(1) the name of the applicant;\n(2) the date of issuance;\n(3) the signature of the director of the department;\n(4) a statement of the kind of equipment to be transported over the highway, the weight and dimensions of the equipment, and the kind and weight of each commodity to be transported; and\n(5) a statement of any condition on which the permit is issued.\n(b) A permit issued under Section 623.071(a) or (b) must also state:\n(1) each highway over which the equipment is to be transported, if the permit is for a single trip; or\n(2) the region or area, as required by rule, over which the equipment is to be operated, if the permit is for other than a single trip.\nActs 1995, 74th Leg., ch. 165, Sec. 1, eff.
