{"data":{"id":"us-ut/utah-code-20a-5-404","jurisdiction":"us-ut","citation":"Utah Code § 20A-5-404","heading":"Election forms -- Preparation and contents.","body":"(1)\n(a) For each election, the election officer shall prepare, for each polling place:\n(i) forms for poll workers to record and verify security seals, ballots cast, and the number of voters who voted; and\n(ii) an official register or pollbook.\n(b) For each election, the election officer shall:\n(i) provide a copy of each form to each of those precincts using paper ballots; and\n(ii) provide a copy of the ballot disposition form and a pollbook to each of those voting precincts using an automated voting system.\n(2) The election officer shall ensure that the forms described in Subsection (1)(a)(i) include:\n(a) a space for the judges to identify:\n(i) the number of ballots voted;\n(ii) the number of registered voters listed in the official register or pollbook; and\n(iii) the total number of voters voting according to the official register or pollbook; and\n(b) a certification, in substantially the following form:\n\"We, the undersigned, judges of an election held at ______ voting precinct, in _______ County, state of Utah, on __________(month\\day\\year), having first been sworn according to law, certify that the information in this form is a true statement of the number and names of the individuals voting in the voting precinct at the election, and that the total number of individuals voting at the election was ____.\n_____________________________\n_____________________________\n_____________________________\nJudges of Election\".\n(3) The election officer shall ensure that the official register or pollbook:\n(a) identifies the voting precinct number on the face of the official register or pollbook; and\n(b) contains:\n(i) a section to record individuals voting on election day; and\n(ii) a section in which to record voters who are challenged.","path":["Title 20A Election Code","Chapter 20A-5 Election Administration","Part 20A-5-4 Election Officer's Duties"],"source_url":"https://le.utah.gov/xcode/Title20A/Chapter5/20A-5-S404.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"2e30bffd5c9da5420f7bf50f11874fee087816643a96e733f785ef7b289c88bb","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-20a-5-403.5","next":"us-ut/utah-code-20a-5-405"},"notice":"GroundRules: Original legal text.
