{"data":{"id":"us-ut/utah-code-26b-1-504","jurisdiction":"us-ut","citation":"Utah Code § 26B-1-504","heading":"Fatality review committees for a resident of the Utah State Hospital or the Utah State Developmental Center.","body":"(1) If a qualified individual who is the subject of a formal review was a resident of the Utah State Hospital or the Utah State Developmental Center, the fatality review coordinator of that facility shall organize a fatality review committee to review the near fatality or the death.\n(2) Except as provided in Subsection (4), a committee described in Subsection (1) shall include the following members:\n(a) the fatality review coordinator for the facility, who shall serve as chair of the committee;\n(b) a member of the management staff of the facility;\n(c) a supervisor of a unit other than the one in which the qualified individual resided;\n(d) a physician;\n(e) a representative from the administration of the division that oversees the facility;\n(f) the department's fatality review coordinator;\n(g) a member of the department's risk management services; and\n(h) a citizen who is not an employee of the department.\n(3) A committee described in Subsection (1) may also include a person whose knowledge or expertise may significantly contribute to the formal review.\n(4) A committee described in this section may not include an individual who:\n(a) was involved in, or who supervises a person who was involved in, the near fatality or the death; or\n(b) has a conflict with the fatality review.","path":["Title 26B Utah Health and Human Services Code","Chapter 26B-1 Department of Health and Human Services","Part 26B-1-5 Fatality Review"],"source_url":"https://le.utah.gov/xcode/Title26B/Chapter1/26B-1-S504.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"f7654a02504d59a039cce6325600847c6d043ca71a486e36f6f9cb46931a8070","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-26b-1-503","next":"us-ut/utah-code-26b-1-505"},"notice":"GroundRules: Original legal text.
