{"data":{"id":"us-ut/utah-code-26b-4-311","jurisdiction":"us-ut","citation":"Utah Code § 26B-4-311","heading":"Content of primary care grant applications.","body":"An applicant for a grant under Section 26B-4-310 shall include, in an application:\n(1) a statement of specific, measurable objectives, and the methods the applicant will use to assess the achievement of those objectives;\n(2) the precise boundaries of the area the applicant will serve, including a description of the medically underserved population the applicant will serve using the grant;\n(3) the results of a need assessment that demonstrates that the population the applicant will serve has a need for the services provided by the applicant;\n(4) a description of the personnel responsible for carrying out the activities of the grant along with a statement justifying the use of any grant funds for the personnel;\n(5) evidence that demonstrates the applicant's existing financial and professional assistance and any attempts by the applicant to obtain financial and professional assistance;\n(6) a list of services the applicant will provide;\n(7) the schedule of fees, if any, the applicant will charge;\n(8) the estimated number of individuals the applicant will serve with the grant award; and\n(9) any other information required by the department in consultation with the committee.","path":["Title 26B Utah Health and Human Services Code","Chapter 26B-4 Health Care - Delivery and Access","Part 26B-4-3 Health Care Access"],"source_url":"https://le.utah.gov/xcode/Title26B/Chapter4/26B-4-S311.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"ef11e7c29f747613253c4747c318138a36384a3873eb4a4bf1a69858843627ec","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-26b-4-310","next":"us-ut/utah-code-26b-4-312"},"notice":"GroundRules: Original legal text.
