{"data":{"id":"us-ut/utah-code-26b-7-316","jurisdiction":"us-ut","citation":"Utah Code § 26B-7-316","heading":"Mandatory reporting requirements -- Contents of reports -- Penalties.","body":"(1)\n(a) A health care provider shall report to the department any case of any person who the provider knows has a confirmed case of, or who the provider believes in his professional judgment is sufficiently likely to harbor any illness or health condition that may be caused by:\n(i) bioterrorism;\n(ii) epidemic or pandemic disease; or\n(iii) novel and highly fatal infectious agents or biological toxins which might pose a substantial risk of a significant number of human fatalities or incidences of permanent or long-term disability.\n(b) A health care provider shall immediately submit the report required by Subsection (1)(a) within 24 hours of concluding that a report is required under Subsection (1)(a).\n(2)\n(a) A report required by this section shall be submitted electronically, verbally, or in writing to the department or appropriate local health department.\n(b) A report submitted pursuant to Subsection (1) shall include, if known:\n(i) diagnostic information on the specific illness or health condition that is the subject of the report, and, if transmitted electronically, diagnostic codes assigned to the visit;\n(ii) the patient's name, date of birth, sex, race, occupation, and current home and work address and phone number;\n(iii) the name, address, and phone number of the health care provider; and\n(iv) the name, address, and phone number of the reporting individual.\n(3) The department may impose a sanction against a health care provider for failure to make a report required by this section only if the department can show by clear and convincing evidence that a health care provider willfully failed to file a report.","path":["Title 26B Utah Health and Human Services Code","Chapter 26B-7 Public Health and Prevention","Part 26B-7-3 Treatment, Isolation, and Quarantine Procedures for Communicable Diseases"],"source_url":"https://le.utah.gov/xcode/Title26B/Chapter7/26B-7-S316.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"8ace1e018a7775d2501eaabd9713361f4024430d853a36429f183d08c202a047","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-26b-7-315","next":"us-ut/utah-code-26b-7-317"},"notice":"GroundRules: Original legal text.
