{"data":{"id":"us-ut/utah-code-31a-22-631","jurisdiction":"us-ut","citation":"Utah Code § 31A-22-631","heading":"Policy summary or illustration.","body":"(1)\n(a) Except as provided in Subsection (1)(b), at the time a life insurance policy is delivered, a policy summary or illustration shall be delivered for the life insurance policy if:\n(i) the life insurance policy includes riders or supplemental benefits, including accelerated benefits; and\n(ii) receipt of benefits under the life insurance policy is contingent upon morbidity requirements.\n(b) In the case of a direct response solicitation, the insurer shall deliver the policy summary or illustration at the sooner of:\n(i) the applicant's request; or\n(ii) at the time of policy delivery regardless of whether the applicant requests a policy summary or illustration.\n(2) In addition to complying with all applicable requirements, the policy summary or illustration shall include:\n(a) a clear and prominent disclosure of how the rider or supplemental benefit interacts with other components of the policy, including deductions from death benefits and policy values;\n(b) an illustration for each covered person of:\n(i) the amount of benefits;\n(ii) the length of benefits; and\n(iii) the guaranteed lifetime benefits, if any;\n(c) a disclosure of the maximum premiums for the rider or supplemental benefit;\n(d) any exclusions, reductions, or limitations on the benefits of the rider or supplemental benefit; and\n(e) if applicable to the policy type:\n(i) a disclosure of the effects of exercising other rights under the policy; and\n(ii) guaranteed maximum lifetime benefits.","path":["Title 31A Insurance Code","Chapter 31A-22 Contracts in Specific Lines","Part 31A-22-6 Accident and Health Insurance"],"source_url":"https://le.utah.gov/xcode/Title31A/Chapter22/31A-22-S631.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"6a394222384916c48b6429337987bc1eed8a46761cea0cdebde55c26191d5898","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-31a-22-630","next":"us-ut/utah-code-31a-22-632"},"notice":"GroundRules: Original legal text.
