{"data":{"id":"us-ut/utah-code-31a-22-639","jurisdiction":"us-ut","citation":"Utah Code § 31A-22-639","heading":"Statement of preauthorization.","body":"(1) An insurer who requires preauthorization or preapproval for coverage under accident and health insurance shall, beginning January 1, 2011, provide an enrollee with a statement of preauthorization if:\n(a) the applicable CPT codes have been submitted to the insurer to determine whether a particular procedure is covered under the terms of the accident and health insurance policy;\n(b) the enrollee has met the requirements for preauthorization of the procedure or encounter; and\n(c) the enrollee requests a statement of preauthorization.\n(2) A statement of preauthorization under Subsection (1) may be sent:\n(a) by mail; or\n(b) electronically.\n(3) A statement of preauthorization shall include a statement that the preauthorization is:\n(a) not a guarantee of payment by an insurer; and\n(b) subject to the policy and contract provisions of the accident and health insurance contract.","path":["Title 31A Insurance Code","Chapter 31A-22 Contracts in Specific Lines","Part 31A-22-6 Accident and Health Insurance"],"source_url":"https://le.utah.gov/xcode/Title31A/Chapter22/31A-22-S639.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"82063b7962dfcf0eb20cd890953bc74d5e39ef13da5fefdf83cb2a366295c71f","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-31a-22-638","next":"us-ut/utah-code-31a-22-641"},"notice":"GroundRules: Original legal text.
