{"data":{"id":"us-ut/utah-code-34a-2-604","jurisdiction":"us-ut","citation":"Utah Code § 34A-2-604","heading":"Employee leaving place of treatment.","body":"(1) An injured employee who desires to leave the locality in which the employee has been employed during the treatment of the employee's injury, or to leave this state, shall:\n(a) report to the employee's attending physician for examination;\n(b) notify the division in writing of the intention to leave; and\n(c) accompany the notice with a certificate from the attending physician setting forth:\n(i) the exact nature of the injury;\n(ii) the condition of the employee; and\n(iii) a statement of the probable length of time disability will continue.\n(2) An employee may leave the locality in which the employee was employed only after:\n(a) complying with Subsection (1); and\n(b) receiving the written consent of the division.\n(3) If an employee does not comply with this section, compensation may not be allowed during the absence.","path":["Title 34A Utah Labor Code","Chapter 34A-2 Workers' Compensation Act","Part 34A-2-6 Medical Evaluations"],"source_url":"https://le.utah.gov/xcode/Title34A/Chapter2/34A-2-S604.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"f227c3c16a0ec3e87821bd362e9b97ef8cec83bf6c832088e6f82dc76b989455","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-34a-2-603","next":"us-ut/utah-code-34a-2-701"},"notice":"GroundRules: Original legal text.
