{"data":{"id":"us-ut/utah-code-53-3-410","jurisdiction":"us-ut","citation":"Utah Code § 53-3-410","heading":"Applicant information required for CDIP and CDL -- State resident to have state CDL.","body":"(1) The application for a CDL, limited-term CDL, or CDIP shall include the following information regarding the applicant:\n(a) full legal name;\n(b) current mailing address;\n(c) Utah residential address;\n(d) physical description, including sex, height, weight, and eye color;\n(e) date of birth;\n(f) documentary evidence of the applicant's valid social security number;\n(g) a complete list of all states in which the applicant was issued a driver license in the previous 10 years upon:\n(i) initial issuance of a Utah license;\n(ii) renewal of a CDL for the first time after September 30, 2002; or\n(iii) transfer of a CDL from another state;\n(h) the applicant's signature;\n(i) evidence of the applicant's lawful presence in the United States by providing documentary evidence:\n(i) that a person is:\n(A) a United States Citizen;\n(B) a United States national; or\n(C) a legal permanent resident alien; or\n(ii) of the applicant's:\n(A) unexpired immigrant or nonimmigrant visa status for admission into the United States;\n(B) pending or approved application for asylum in the United States;\n(C) admission into the United States as a refugee;\n(D) pending or approved application for temporary protected status in the United States;\n(E) approved deferred action status;\n(F) pending application for adjustment of status to legal permanent resident or conditional resident; or\n(G) conditional permanent resident alien status; and\n(j) beginning on January 30, 2012, a medical certification status.\n(2) An application under this section shall also include all certifications required by 49 C.F.R., Part 383.71.\n(3) When the holder of a license under this part changes the holder's name, mailing address, or residence, the holder shall make application for a duplicate license within 30 days of the change.\n(4) A person who has been a resident of this state for 30 consecutive days may not drive a commercial motor vehicle under the authority of a commercial driver license issued by another jurisdiction.","path":["Title 53 Public Safety Code","Chapter 53-3 Uniform Driver License Act","Part 53-3-4 Uniform Commercial Driver License Act"],"source_url":"https://le.utah.gov/xcode/Title53/Chapter3/53-3-S410.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"99c7ad8bcaaee5b59c6c79bafe2083e1f06730178a6872eda5abbe5a4016e14d","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-53-3-409","next":"us-ut/utah-code-53-3-410.1"},"notice":"GroundRules: Original legal text.
