{"data":{"id":"us-ut/utah-code-58-17b-308","jurisdiction":"us-ut","citation":"Utah Code § 58-17b-308","heading":"Term of license -- Expiration -- Renewal.","body":"(1)\n(a) Except as provided in Subsection (2), the division shall issue a license for a term of two years as the division establishes by rule the division makes in accordance with Title 63G, Chapter 3, Utah Administrative Rulemaking Act.\n(b) The division may extend or shorten a license term by up to one year to stagger renewals as the division determines by rule in accordance with Title 63G, Chapter 3, Utah Administrative Rulemaking Act.\n(c) A license expires on the expiration date shown on the license.\n(2) The duration of a pharmacy intern license may be no longer than:\n(a) two years for a license issued under Subsection 58-17b-304(6)(b); or\n(b) five years for a license issued under Subsection 58-17b-304(6)(a).\n(3) A pharmacy intern license issued under this chapter may not be renewed, but the division may extend the pharmacy intern license in collaboration with the board.\n(4) As a prerequisite for renewal of a class D pharmacy license of a pharmacy that engages in compounding, a licensee shall submit the most recent inspection report:\n(a) conducted within two years before the application for renewal; and\n(b)\n(i) conducted as part of the National Association of Boards of Pharmacy Verified Pharmacy Program; or\n(ii) performed by the state licensing agency of the state in which the applicant is a resident and in accordance with the National Association of Boards of Pharmacy multistate inspection blueprint program.","path":["Title 58 Occupations and Professions","Chapter 58-17b Pharmacy Practice Act","Part 58-17b-3 Licensing"],"source_url":"https://le.utah.gov/xcode/Title58/Chapter17b/58-17b-S308.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"b769f9f3a606fd8a5868519f4e2941102b244cc08694ed4eb1115d9d9efd77f5","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-58-17b-306","next":"us-ut/utah-code-58-17b-309"},"notice":"GroundRules: Original legal text.
