{"data":{"id":"us-ut/utah-code-6-2-112","jurisdiction":"us-ut","citation":"Utah Code § 6-2-112","heading":"Proof of claim.","body":"(1) A proof of claim must:\n(a) state the name, address, and other contact information reasonably necessary to communicate with the creditor;\n(b) state the amount of the claim;\n(c) briefly state the nature of the claim;\n(d) identify any asset of the assignment estate securing the claim;\n(e) be signed by the creditor under penalty of perjury;\n(f) include a copy of a record, if any, on which the claim is based;\n(g) be submitted using the method established under Subsection 6-2-108(2)(e); and\n(h) be submitted by the date established by the assignee under Subsection 6-2-108(2)(f).\n(2) A proof of claim submitted in compliance with this section is prima facie evidence of the validity and amount of the claim.\n(3) The submission by a creditor of a proof of claim in compliance with this section constitutes the creditor's:\n(a) consent to the jurisdiction of a court with jurisdiction in accordance with Section 6-2-120; and\n(b) assignment to the assignee of any right of the creditor to bring a voidable transaction action relating to the creditor's claim.","path":["Title 6 Assignment for Benefit of Creditors","Chapter 6-2 Uniform Assignment for Benefit of Creditors"],"source_url":"https://le.utah.gov/xcode/Title6/Chapter2/6-2-S112.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:33Z","sha256":"15031c74a320dcd38af81629b7c17a8df0be34f59ddc207de1a5eb617272b805","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-6-2-111","next":"us-ut/utah-code-6-2-113"},"notice":"GroundRules: Original legal text.
