{"data":{"id":"us-ut/utah-code-78b-7-207","jurisdiction":"us-ut","citation":"Utah Code § 78B-7-207","heading":"Forms and assistance -- No fees.","body":"(1) The Administrative Office of the Courts shall adopt and make available uniform forms for petitions and orders conforming to this part. The forms shall notify the petitioner that:\n(a) a knowing falsehood in any statement under oath may subject the petitioner to felony prosecution;\n(b) the petitioner may provide a copy of the order to the principal of the minor's school; and\n(c) the petitioner may enforce a court order through the court if the respondent violates or fails to comply with a provision of the order.\n(2) If the petitioner is not represented, the clerk of the court shall provide, directly or through an agent:\n(a) the forms adopted pursuant to Subsection (1);\n(b) clerical assistance in completing the forms and filing the petition;\n(c) information regarding means for service of process;\n(d) a list of organizations with telephone numbers that may represent the petitioner; and\n(e) information regarding the procedure for transporting a jailed or imprisoned respondent to hearings, including transportation order forms when necessary.\n(3) No fee may be imposed by a court, constable, or law enforcement agency for:\n(a) filing a petition under this chapter;\n(b) obtaining copies necessary for service or delivery to law enforcement officials; or\n(c) service of a petition, ex parte child protective order, or child protective order.","path":["Title 78B Judicial Code","Chapter 78B-7 Protective Orders and Stalking Injunctions","Part 78B-7-2 Child Protective Orders"],"source_url":"https://le.utah.gov/xcode/Title78B/Chapter7/78B-7-S207.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:34Z","sha256":"0799bafe15f6541cc72468ce90f5e27b80706ef14b6e073a52fdd4d3fe8d488e","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-78b-7-206","next":"us-ut/utah-code-78b-7-301"},"notice":"GroundRules: Original legal text.
