{"data":{"id":"us-ut/utah-code-81-12-105","jurisdiction":"us-ut","citation":"Utah Code § 81-12-105","heading":"Contents of petition.","body":"(1)\n(a) A petition under this chapter must be verified and include a copy of any existing child custody determination, if available.\n(b) The petition must specify the risk factors for abduction, including the relevant factors described in Section 81-12-106.\n(2) Subject to Subsection 81-11-209(5), if reasonably ascertainable, the petition must contain:\n(a) the name, date of birth, and gender of the minor child;\n(b) the customary address and current physical location of the minor child;\n(c) the identity, customary address, and current physical location of the respondent;\n(d) a statement of whether a prior action to prevent abduction or domestic violence has been filed by a party or other individual or entity having custody of the minor child, and the date, location, and disposition of the action;\n(e) a statement of whether a party to the proceeding has been arrested for a crime related to domestic violence, stalking, or child abuse or neglect, and the date, location, and disposition of the case; and\n(f) any other information required to be submitted to the court for a child custody determination under Section 81-11-209.","path":["Title 81 Utah Domestic Relations Code","Chapter 81-12 Uniform Child Abduction Prevention Act"],"source_url":"https://le.utah.gov/xcode/Title81/Chapter12/81-12-S105.html","current_through":"2026 General Session","vintage":"","retrieved_at":"2026-09-03T11:34:34Z","sha256":"522f8e1c08f2d2c2faf9056a7a195ad797265bb11d880e7709d59e117af4aad1","source_id":"us-ut","stale":false,"prev":"us-ut/utah-code-81-12-104","next":"us-ut/utah-code-81-12-106"},"notice":"GroundRules: Original legal text.
