A description of the applicant's management plan, with budget and identification of management personnel by function, job descriptions and qualifications for each management position, and a copy of the organization chart;\n\n2. Management personnel to the extent known and with respect to each:\n\na. Legal name, alias or aliases and previous name or names;\n\nb. Current residence and business addresses and telephone numbers;\n\nc. Qualifications and experience in the following areas:\n\n(1) General business;\n\n(2) Marketing, promotion and advertising;\n\n(3) Finance and accounting;\n\n(4) Horse racing;\n\n(5) Pari-mutuel wagering;\n\n(6) Security; and\n\n(7) Human and animal health and safety.\n\nd. Description of the terms and conditions of employment and a copy of each type of agreement;\n\n3. Consultants and other contractors who have provided or will provide management-related services to applicant with respect to each:\n\na. Full name;\n\nb. Current address and telephone number;\n\nc. Nature of services;\n\nd. Qualifications and experience; and\n\ne. Description of terms and conditions of each contractor's agreement and a copy of the agreement.\n\n4. Memberships of the applicant, management personnel and consultants in horse racing organizations.\n\n5. Description of the applicant's marketing, promotion and advertising plans;\n\n6. A description of the applicant's plan for concessions, including whether the licensee will operate concessions and, if not, who will;\n\n7. A description of training of the applicant's personnel; and\n\n8. A description of plans for compliance with all laws pertaining to discrimination, equal employment and affirmative action; policies regarding recruitment, use and advancement of minorities; policies with respect to minority contracting; and a copy of Equal Employment Opportunity Statement.","path":["Title 11.
