What action or adverse determination the agency intends to take and the effective date of such action;\n\n2.  A clear statement of the specific reasons  supporting the intended action or adverse determination;\n\n3. The specific regulations that support or the change in law that requires the action or adverse determination;\n\n4. The right to request an evidentiary hearing, and the methods and time limits for doing so;\n\n5. The right to request an expedited evidentiary hearing;\n\n6.
