The Community Living Waiver is the developmental disabilities waiver designed particularly to support those individuals who require some form of a residential service 24 hours per day, seven days per week.\n\nB. The services covered in the Community Living Waiver are:\n\n1. Assistive technology service (12VAC30-122-270).\n\n2. Benefits planning service (12VAC30-122-280).\n\n3. Center-based crisis support service (12VAC30-122-290).\n\n4. Community-based crisis support service (12VAC30-122-300).\n\n5. Community coaching service (12VAC30-122-310).\n\n6. Community engagement service (12VAC30-122-320).\n\n7. Community guide service (12VAC30-122-330).\n\n8. Companion service (12VAC30-122-340).\n\n9. Crisis support service (12VAC30-122-350).\n\n10. Electronic home-based support service (12VAC30-122-360).\n\n11. Environmental modifications service (12VAC30-122-370).\n\n12. Group day service (12VAC30-122-380).\n\n13. Group home residential service (12VAC30-122-390).\n\n14. Group and individual supported employment service (12VAC30-122-400).\n\n15. In-home support service (12VAC30-122-410).\n\n16.  Employment and community transportation service (12VAC30-122-440).\n\n17. Peer  mentor supports service (12VAC30-122-450).\n\n18. Personal assistance service (12VAC30-122-460).\n\n19. Personal emergency response system service (12VAC30-122-470).\n\n20. Private duty nursing service (12VAC30-122-480).\n\n21. Respite service (12VAC30-122-490).\n\n22. Services facilitation service (12VAC30-122-500).\n\n23. Shared living  service (12VAC30-122-510).\n\n24. Skilled nursing service (12VAC30-122-520).\n\n25. Sponsored residential service (12VAC30-122-530).\n\n26. Supported living residential service (12VAC30-122-540).\n\n27. Therapeutic consultation service (12VAC30-122-550).\n\n28. Transition service (12VAC30-122-560).\n\n29. Workplace assistance service (12VAC30-122-570).\n\nC. Services shall be rendered in compliance with all of the requirements set out in 12VAC30-122-120. Providers claims for reimbursement shall be supported by  documentation in the record in accordance with federal requirements and DMAS regulatory requirements.
