NFs shall not accept handwritten LTSS screening forms as proof that admission criteria have been met and documented.\n\nThe NF LTSS screening team shall be responsible for screening individuals admitted directly from a hospital for skilled nursing or rehabilitation not covered by the Commonwealth's program of medical assistance and having a change in level of care requiring LTSS.","path":["Title 12.
