The provider's policy regarding admission shall at a minimum require the individual to:\n\n1. Meet diagnostic criteria for a moderate or severe substance use or addictive disorder as defined by the DSM; and\n\n2. Meet the admission criteria of Level 3.1 of ASAM.","path":["Title 12.
