The provider shall develop a method for documenting the provision of crisis intervention and  emergency services. Documentation shall include the following:\n\n1. Date and time;\n\n2.  Description of the nature of or circumstances surrounding the crisis or emergency;\n\n3. Name of individual;\n\n4.  Description of precipitating factors;\n\n5.  Interventions or treatment provided;\n\n6.  Names of employees or contractors  responding to or consulted during the crisis or emergency; and\n\n7. Outcome.\n\nB. If a crisis or  emergency involves an individual who is admitted into service, documentation of the crisis intervention  or provision of emergency services shall become part of his record.","path":["Title 12.
