The provider shall maintain the following emergency medical information for each individual:\n\n1. If available, the name, address, and telephone number of:\n\na. The individual's physician; and\n\nb.  The  authorized representative  or other person to be notified;\n\n2. Medical insurance company name and policy or Medicaid, Medicare , or CHAMPUS number, if any;\n\n3. Currently prescribed medications and over-the-counter medications used by the individual;\n\n4. Medication and food allergies;\n\n5. History of substance abuse;\n\n6. Significant medical problems or conditions;\n\n7. Significant ambulatory or sensory problems;\n\n8. Significant communication problems; and\n\n9. Advance directive, if one exists.\n\nB.
