Achieved the goals of the treatment services and no longer require medication for opioid use disorder treatment level of care;\n\n2. Been unable to achieve the goals of the individual's treatment but could achieve the individual's goals with a different type of treatment; or\n\n3. Achieved the individual's original treatment goals but have developed new treatment challenges that can only be adequately addressed in a different type of treatment.","path":["Title 12.
