The provider shall implement written procedures for promptly:\n\n1. Providing or arranging for the provision of medical and dental services for health problems identified at admission;\n\n2. Providing or arranging for the provision of routine ongoing and follow-up medical and dental services after admission;\n\n3. Providing emergency services for each resident; and\n\n4. Providing emergency services for any resident in crisis, including procedures for crisis or clinical stabilization and immediate access to appropriate internal and external resources, including a provision for obtaining physician and mental health clinical services.\n\nB. The following written information concerning each resident shall be readily accessible to staff who may have to respond to a medical or dental emergency:\n\n1. Name, address, and telephone number of the physician and dentist to be notified;\n\n2. Name, address, and telephone number of a relative or other person to be notified;\n\n3. Medical insurance company name and policy number or Medicaid number;\n\n4. Information concerning:\n\na. Use of medication;\n\nb. All allergies, including medication allergies;\n\nc. Substance abuse and use; and\n\nd. Significant past and present medical problems; and\n\n5. Written permission for emergency medical care, dental care, and obtaining immunizations or a procedure and contacts for obtaining consent.","path":["Title 12. Health","Agency 35.
