The provider shall develop and implement a method for documenting the provision of crisis intervention and clinical emergency services. Documentation shall include:\n\n1. Date and time;\n\n2. Nature of crisis or emergency;\n\n3. Name of resident;\n\n4. Precipitating factors;\n\n5. Interventions or treatment provided;\n\n6. Employees or contractors involved;\n\n7. Outcome; and\n\n8. Any required follow-up.\n\nB. If a crisis or clinical emergency involves a resident who receives medical or mental health services, the crisis intervention documentation shall become part of the resident's record.","path":["Title 12.
