The reasons for closure of a client to the program shall be in accordance with existing department eligibility guidance documents as stated in 12VAC5-191-70.\n\nB. If, at the time of closure to the program, the client still needs medical care for the condition for which he was in the program and for the service that he has been receiving from the program, the client will be referred to another source.  The services may terminate only following notice to the client that such services will be terminated. Medical care services cannot be terminated for clients receiving ongoing care without making a good faith effort to secure alternative care.","path":["Title 12. Health","Agency 5.
