If the hospice program contracts for services, there shall be a written agreement for the provision of those services.\n\nB. The written agreement shall include, but is not limited to:\n\n1. The services to be furnished by each party to the contract;\n\n2. The contractor's responsibility for participating in developing plans of care;\n\n3. The manner in which services will be controlled, coordinated, and evaluated by the hospice program;\n\n4. The procedures for submitting clinical and progress notes, scheduling of visits, and periodic patient evaluation;\n\n5. The process for payment for services furnished under the contract; and\n\n6. Adequate indemnity coverage.\n\nC. The hospice program shall have procedures for providing patient services in the event the contractor is unable to comply with the plan of care.\n\nD. The contractor shall conform to applicable hospice program policies and procedures as specified in the contract, including the required sworn statement and criminal record check.","path":["Title 12.
