In the event of a medication error or adverse drug reaction, employees shall promptly notify the patient's physician, the medical director, the nurse and the patient's family and shall take action as directed. Adverse outcomes of medication errors or drug reactions shall also be reported to the OLC within 48 consecutive hours of the event.\n\nB. Actions taken shall be documented in the patient's record.\n\nC.
