Following any period of institutional control needed to meet the requirements found necessary under 12VAC5-481-2410, the licensee may apply for an amendment to terminate the license.\n\nB. This application will be reviewed in accordance with the provisions of  12VAC5-481-450.\n\nC. A license shall be terminated only when the agency finds:\n\n1. That the institutional control requirements found necessary under  12VAC5-481-2410 8 have been met;\n\n2. That any additional requirements resulting from new information developed during the institutional control period have been met; and\n\n3.
