Operating and emergency procedures;\n\n2. Survey records required pursuant to 12VAC5-481-3340 for the period of operation at the site;\n\n3. Evidence of current calibration for the radiation survey instruments in use at the site;\n\n4. When operating in the state under reciprocity, a copy of the appropriate license, certificate of registration, or equivalent document(s); and\n\n5.
