To facilitate the process, applicants  shall submit  payment statements from each lending institution.\n\nParticipants who have consolidated qualifying loans with other loans  shall submit on request other documentation, such as copies of original loan applications, to verify the portion of the loan that qualifies for repayment.\n\nThe applicant shall submit all requested loan documentation prior to approval by the  department.","path":["Title 12.
