The commissioner shall establish a schedule of fees to be charged by the department for services related to construction, maintenance, and repair or replacement of onsite sewage disposal systems, alternative discharge systems, and private wells and for appeals before the Review Board.\n\nB. In establishing fees, the commissioner shall consider the actual or estimated average cost to the agency of delivering each service included in the schedule of fees.\n\nThe following fee schedule is hereby established:\n\nSCHEDULE OF FEES\n\nApplication or Service\n\nFee\n\nCertification letter, no onsite soil  evaluator/professional engineer (OSE/PE) documentation (no charge for well)\n\n$350\n\nCertification letter with OSE/PE documentation, ≤1,000  gpd\n\n$320\n\nCertification letter with OSE/PE documentation, \u003e1,000  gpd\n\n$1,400\n\nConstruction permit for treatment works only, no OSE/PE  documentation\n\n$425\n\nCombined well and treatment works construction permit, no  OSE/PE documentation\n\n$725\n\nCombined well and treatment works construction permit with  OSE/PE documentation, ≤1,000 gpd\n\n$525\n\nConstruction permit for treatment works only with OSE/PE  documentation, ≤1,000 gpd\n\n$225\n\nConstruction permit for treatment works only with OSE/PE  documentation, \u003e1,000 gpd\n\n$1,400\n\nCombined well and treatment works construction permit with  OSE/PE documentation, \u003e1,000 gpd\n\n$1,700\n\nPrivate well construction or abandonment permit, with or  without OSE/PE documentation\n\n$300\n\nClosed-loop geothermal well system (one fee per well system)\n\n$300\n\nAlternative discharge system inspection fee\n\n$75\n\nMinor modification to an existing system\n\n$100\n\nAppeal before the Review Board\n\n$135","path":["Title 12.
