The board has established the following fees applicable to the certification of sex offender treatment providers:\n\nRegistration of supervision\n\n$50\n\nAdd or change supervisor\n\n$25\n\nApplication processing and initial certification fee\n\n$90\n\nCertification renewal\n\n$75\n\nDuplicate certificate\n\n$5\n\nLate renewal\n\n$25\n\nReinstatement of an expired certificate\n\n$125\n\nReplacement of or additional wall certificate\n\n$15\n\nHandling fee for returned check or  dishonored credit card or debit card\n\n$50\n\nReinstatement following revocation or suspension\n\n$500\n\nOne-time reduction in fee for renewal on June 30, 2020\n\n$55\n\nB.
