The application fee for reinstatement of a license will be the amount provided for in 18VAC155-20-40 B 4. An individual who has not been reinstated within two years after expiration of the license must reapply as a new applicant. The new applicant must provide evidence of satisfactory completion of the training course required by this chapter and pass the examination as determined by the board.","path":["Title 18.
