No license issued by the board  may be assigned or otherwise transferred.\n\nB. A licensee must report, in writing, to the board any change of the following:\n\n1. The licensee's legal name. Such report must be accompanied by documentation acceptable to the board that verifies the name change.\n\n2. The licensee's address, to include the physical address when applicable.\n\nC. A change of name or address must be reported to the board within 60 days of the change. The board is not responsible for the licensee's failure to receive notices, communications, and correspondence caused by the licensee's failure to report to the board any change of name or address.","path":["Title 18.
