The course being offered no longer meets the standards established by the board.\n\n2. The provider, through an agent or otherwise, advertises its services in a fraudulent or deceptive way.\n\n3. The provider, instructor, contact person, or designee of the provider falsifies any information relating to the application for approval, course information, or participant records.\n\n4. The training course provider fails to provide the participant with documentation of course completion showing the hours completed, the date of training, and the training course identification number assigned by the board.\n\n5. There is a change in the information regarding the training course, if the change results in noncompliance with this part.\n\n6. The provider fails to comply with 18VAC160-40-415.\n\n7.
