In addition to the appropriate fee provided in 18VAC48-50-60, the application shall include but is not limited to:\n\n1. The name of the provider;\n\n2. Provider contact person, address, and telephone number;\n\n3. Program contact hours;\n\n4. Schedule of training program, if established, including dates, times, and locations;\n\n5. Instructor information, including name, license  or certificate number(s), if applicable, and a list of trade-appropriate designations, as well as a professional resume with a summary of teaching experience and subject-matter knowledge and qualifications acceptable to the board;\n\n6. A summary of qualifications and experience in providing training  under this chapter;\n\n7. Training program and material fees; and\n\n8. Training program syllabus.","path":["Title 18.
