The following fees shall apply for late renewal of licensure or registration up to one year following expiration:\n\n1. Surface transportation and removal service registration\n\n$100\n\n4. Courtesy card\n\n$100\n\n5. Crematory\n\n$75\n\n6. Waiver of full-time manager requirement\n\n$35\n\n7. Inactive funeral service, funeral director, or embalmer license\n\n$40\n\nD. The following fees shall apply for reinstatement of licensure or registration:\n\n1. License to practice funeral service or as a funeral director or an embalmer\n\n$400\n\n2. Establishment license\n\n$635\n\n3. Surface transportation and removal service registration\n\n$425\n\n4. Courtesy card\n\n$425\n\n5. Crematory\n\n$275\n\n6. Reinstatement following suspension\n\n$1,000\n\n7. Reinstatement following revocation\n\n$2,000\n\nE. Other fees.\n\n1. Change of manager or establishment name\n\n$100\n\n2. Verification of license or registration to another state\n\n$50\n\n3. Duplicate license, registration, or courtesy card\n\n$25\n\n4. Duplicate wall certificates\n\n$60\n\n5. Change of ownership\n\n$100\n\n6. Nonroutine reinspection (i.e., structural change to preparation room, change of location or ownership)\n\n$400\n\nF. Fees for approval of continuing education providers.\n\n1. Application or renewal for continuing education provider\n\n$400\n\n2. Late renewal of continuing education provider approval\n\n$100\n\n3.
