Each prescription for a Schedule II through V drug shall bear the name of the  patient care team physician or podiatrist and of the physician assistant.\n\nB. The physician assistant shall disclose to the patient that he is a licensed physician assistant, and also the name, address and telephone number of the  patient care team physician or podiatrist.
