Violates any provision of applicable laws or regulations;\n\n2. Engages in conduct or practices that are in violation of statutes related to abuse or neglect of children;\n\n3. Deviates significantly from the program or services for which a license was issued without obtaining prior written approval from the department or fails to correct such deviations within the specified time; or\n\n4. Engages in a willful action or gross negligence that jeopardizes the care or protection of residents.\n\nB. If revocation of a license is recommended, the facility shall be notified in writing of the noncompliances, the proposed action, the right to appeal, and the appeal process.","path":["Title 22.
