The commissioner notifies the requester, or other adult if applicable, and the sponsor of his approval or denial in writing by certified mail.\n\nB. Any approved waiver is for a specific person and a specific facility and must include:\n\n1. Name of individual;\n\n2. Name of facility;\n\n3. Effective dates;\n\n4. Terms, conditions, and stipulations, if any;\n\n5. Criminal conviction for which the waiver was granted;\n\n6. Date of criminal conviction;\n\n7. Relevant court and location;\n\n8.
