The date of receipt of each application for such payment (Library Forms RW-65A(1), RW-65B (1) and RW-65C(1));\n\n2. The date on which each payment was made or the application rejected (Parcel File);\n\n3. Supporting data showing how the amount of the supplemental payment to which the applicant is entitled was calculated (Library Form RW-62A or RW-62B);\n\n4. A copy of the closing statement to support when replacement housing is purchased (Parcel File);\n\n5. A copy of the Truth in Lending Statement and other data, including computations to support the increased interest payment (Parcel File and Library Form RW-66);\n\n6. The individual responsible for determining the amount of the replacement housing payment shall place in the file a signed and dated statement setting forth:\n\na. The amount of the replacement housing payment (Library Forms RW-62A and B);\n\nb. An understanding that the determined amount is to be used in connection with a federal-aid highway project (Library Forms RW-62A and B); and\n\nc. There is no direct or indirect present or contemplated personal interest in this transaction nor will any benefit be derived from the replacement housing payment (Library Forms RW-62A and B); and\n\n7. A statement that the relocated person has been relocated into decent, safe and sanitary replacement housing (Library Form RW-69B).","path":["Title 24. Transportation And Motor Vehicles","Agency 30.
